**In order to verify coverage through Carelon, please check the back of your insurance card to ensure that behavioral health benefits are indicated to Carelon. Your primary medical plan will be listed on the front of the card, while outsourcing any behavioral health benefits will be listed on the back, if not provided directly through your insurance plan. 

No Insurance - No Problem

If you prefer to have complete separation from your insurance provider or do not have insurance you are still able to be seen at the out of pocket rates indicated for the state you live in.
